Our service

From a one-off session to regular check-ins, we can offer you flexible, one-to-one support according to your needs and aspirations;

  • A safe and supportive space to talk about how you’re feeling

  • Practical support with form filling and writing applications for benefits and services

  • Understanding your rights and entitlements

  • Support with identifying and navigating services, work and training opportunities

What can I expect?

What we offer: Pathways can offer a supportive space to talk about how you’re feeling; help you to identify, prioritise and break down your goals for life or wellbeing so they feel more manageable; offer practical support with anything from form-filling to applying for benefits and other services; and help you to find and connect with the right services and opportunities for you.

How long for: The number of sessions you have will depend on your needs and outstanding practical goals; and an agreement between you/your support worker to continue to work together. We can offer up to 12 sessions and will regularly reflect on and review progress against your goals.

Who our service is for: Our service is for 18-25-year-olds living in, or registered with a GP in, City and Hackney. We can best support individuals who are struggling with low wellbeing, isolation, and/or issues such as mild-moderate depression and anxiety; are not tied in with other services; and are feeling stuck and need help planning their next steps around a particular issue or goal.

As the focus of our support is very practical and time limited, if your needs are more complex, we will invite you to an informal assessment to see if we are best placed to support you. If not, we can signpost you to another service that may be better placed to meet your needs.

Please note that this is not a therapeutic or crisis service. For someone to talk to immediately, you can call the Samaritans 24/7 for free on 116 123 or text SHOUT to 85258. If you are in crisis, you can attend your nearest A&E. Homerton Hospital A&E (Tel: 020 8510 5555) offers access to support 24/7, and specialises in the assessment, treatment and management of mental health problems.

How can I apply? You can apply through our online self-referral form at the top of this page. Just hit the purple button on the left and fill out the form.
